Key Responsibilities

• Evaluate speech, language, cognitive-communication, and swallowing disorders in home-based patients
  
• Develop individualized treatment plans based on physician orders and clinical assessments
  
• Provide direct speech therapy services to help patients regain or improve communication and swallowing abilities
  
• Educate patients and family members on exercises and strategies for ongoing progress
  
• Document patient progress, update care plans, and coordinate with interdisciplinary team members
  
• Participate in care conferences, in-service training, and performance improvement activities
  
• Maintain compliance with HIPAA and company policies for patient safety and confidentiality

What We’re Looking For

• Master’s degree in Speech-Language Pathology
  
• Current SLP license in the state of New Mexico
  
• Certificate of Clinical Competence (CCC-SLP) from ASHA
  
• Prior experience in home health or medical settings preferred
  
• Strong interpersonal and organizational skills
  
• Self-starter who thrives in an independent, mobile care environment
  
• Valid driver’s license and reliable transportation
