What You’ll DoAs an LPN across our Residential and Day Habilitation programs, you’ll be a vital healthcare champion for the individuals we serve. You will deliver quality direct care, support daily health routines, and collaborate with a dedicated team in a dynamic community setting.
- Provide Compassionate Nursing Care: Monitor health conditions, perform assessments, and carry out individualized health plans.
- Manage Medication Administration: Administer medications accurately and maintain full compliance with MassHealth, DPH, and DDS MAP standards.
- Support Team Scheduling: Help maintain program coverage by coordinating shifts with internal staff and agency partners.
- Collaborate Interdisciplinarily: Partner with RNs, physicians, residential leaders, guardians, and state service coordinators to ensure seamless care.
- Guide Direct Care Staff: Educate and support program staff on individualized care needs, safety protocols, and emergency procedures.
- Maintain Quality Records: Keep clear, precise electronic health records (EHR) to keep our team organized and audit-ready.
- Education & Credentials: Valid Massachusetts LPN License (Required)
- Certifications: Active CPR/First Aid Certification (Required); MAP certification/familiarity preferred
- Experience: Minimum 1–2 years of clinical nursing experience; background supporting individuals with intellectual/developmental disabilities (ID/DD) or in DDS/Day Hab settings is a huge plus!
- Key Skills: Strong organization, team player attitude, reliable communication, and comfort using electronic health records (EHR).
