Complete Health is seeking a highly analytical, detail-oriented Population Health Analyst to join our team and help drive meaningful improvements in patient care, operational performance, and value-based healthcare initiatives.

Position Summary

The Population Health Analyst works collaboratively with clinical and operational leaders to support the organization's population health, quality reporting, and data analytics initiatives. This role is responsible for ensuring the accuracy, integration, validation, and utilization of clinical, operational, and claims data to support decision-making, regulatory reporting, quality improvement efforts, and patient care outcomes.

This position plays a key role in developing reports and dashboards, monitoring quality measures, managing patient empanelment processes, identifying care gaps, and supporting value-based care programs.

What You'll Do

  • Analyze and validate clinical, operational, and claims data from multiple sources.
  • Develop reports, dashboards, provider scorecards, and performance metrics to support organizational goals.
  • Monitor quality measures including UDS, preventive care, chronic disease management, and payer quality programs.
  • Support value-based care initiatives through data analysis, reporting, and performance monitoring.
  • Conduct data validation audits and reconcile discrepancies across systems.
  • Analyze claims data to identify trends, utilization patterns, reimbursement opportunities, and quality improvement opportunities.
  • Develop and maintain patient registries and population health reports to identify high-risk patients and gaps in care.
  • Support patient empanelment and provider panel management activities.
  • Conduct schedule scrubbing to identify patients due for preventive services, chronic disease follow-up, and quality measure compliance.
  • Collaborate with clinical leaders to implement data-driven improvement strategies that enhance patient outcomes and access to care.
  • Provide technical support and education related to reporting tools, software applications, and data workflows.

Requirements:
  • Associate degree or equivalent experience
  • Strong analytical, organizational, and problem-solving skills.
  • Advanced Microsoft Excel and reporting capabilities.

Preferred Qualifications

  • Bachelor's degree in Health Informatics, Healthcare Administration, Public Health, Data Analytics, Information Systems, or a related field.
  • Three years of experience in healthcare analytics, quality reporting, population health, healthcare operations, or health information management.
  • Experience with electronic health record systems, population health platforms, and payer reporting systems.
  • Knowledge of UDS reporting, quality measures, claims data, value-based care, patient empanelment, and provider attribution methodologies.
  • Experience developing dashboards, executive reports, and performance scorecards.
