Job Description
Position Summary
Responsible for managing the day-to-day operations in the home; caregiver recruitment and retention, scheduling, training, and supervising staff; and assisting the Administrator with other duties as needed.
Responsibilities
- Promotes Harvest Home Senior Living Services mission and philosophy in daily work.
- Ensures that residents receive quality care in accordance with the state and federal regulations governing long term care.
- Provides training and oversight to caregivers in the provision of medication assistance, health monitoring, treatments as ordered, and resident personal cares.
- Provides training and oversight to ensure appropriate infection
- Communicates with physicians, pharmacy, therapy, hospice, home health and any other 3rd party agency regarding resident medications, treatment, and concerns.
- Participates in the admission process for new residents, including pre-admission assessments, communication with family, care plans, communication with caregivers and documentation of necessary forms
- Completes resident change of condition and discharge according to company protocol.
- Documents in resident charts in a timely manner regarding any changes or communication with family, physicians, and other third party groups.
- Schedules and completes ISP reviews with resident’s family/POA
- Maintains resident immunization records including Covid, influenza, and pneumonia vaccinations.
- Monitors resident vitals as documented weekly by caregivers, notifies MD as needed and documents data in resident chart.
- Manages resident appointments and posts on house calendar, Google Calendar and communication log for sta ff
- Responds to resident or staff concerns and reports them to other administrative team members as appropriate
- Completes resident service schedules and updates as needed with assistance from caregivers and in accordance with the resident care plans.
- Updates resident care plans as needed and communicating those changes to caregivers.
- Performs regular resident chart audits
- Reviews and checks in all resident monthly meds
- Reviews all MD orders and transcribes to resident MARS
- Communicates with pharmacy timely for any needed medication refills.
- Ensures that med rooms and med carts are organized and clean.
- Destroys medications per protocol with assistance from caregivers.
- Maintains first aid supplies
- Performs routine medication administration audits
- Reads communication logs daily and removes older notes
- Documents necessary communication for staff in communication log
- Scheduled on-call rotation after hours and on weekends/holidays to provide caregiver support
- Participates in opportunities for professional growth and career enhancement
- Performs all other duties as assigned.

- Computer skills, with the ability to learn customized programs.

- Ability and willingness to work flexible hours to meet the requirements of the job and being on-call (to be shared).
All General Staffing Requirements:
1. Free of communicable disease
2. Able to physically/mentally perform tasks, as verified by a health screening that includes TB clearance
3. Criminal record clearance and background check as required by Wisconsin Bureau of Quality Assurance
4. Current First Aid certification
5. Ability to read, write, and understand English
6. General understanding of the needs of the elderly and client specific needs per Harvest Home Senior Living Services license
7. Willingness to learn
Education & Experience
Must be at least 21 years of age. Must have a current LPN license for the state of Wisconsin and be a graduate form an accredited school of professional nursing. At least one year of nursing experience required.
