SDPI Coordinator Needed in Arizona

Summary

Client Job Description

Client Job Description:

We are seeking a skilled and compassionate Healthcare provider who understands and respects the values of our tribal communities. Together, staff will provide care that meets the needs of the community and offers a meaningful and culturally responsive experience for both patients and their families.

Specialty: SDPI (Special Diabetes Program for Indians)

Work Setting: Clinic may also assist with mobile unit and assist with in-home visits

Schedule: Monday Friday, 40 hours per week

Facility Type: Critical access hospital

Requirements:

Licensure: unrestricted RN license from AZ or other compact state OR Registered Dietitian (RD) or Registered Dietician Nutritionist (RDN).

Education: Bachelor's of Nursing (BSN)

Experience required: 2 years of diabetes care and 2 years providing direct care to patients; preferring 2 years of case management and/or care coordination.

Life Support Certifications: BLS, ACLS, strongly preferring Certified Diabetes Care and Education Specialist (CDCES) credential
