Job Overview:

The Advanced Practice Nurse (APN) and Patient Navigator (PN) is an integrated clinical and care-coordination role within the General Obstetrics and Gynecology division.

The APN provides comprehensive, evidence-based care across the spectrum of preventive, gynecologic, and obstetric services, including assessment, diagnosis, treatment, procedures within scope, and prescriptive management in accordance with New Jersey Board of Nursing regulations and Rutgers Health policies.

The PN function streamlines access and continuity of care by coordinating appointments, authorizations, testing, referrals, and transitions of care; advancing health literacy; addressing barriers and social determinants of health; and serving as the liaison among patients, families, and the interprofessional team.

Together, the combined role improves clinical outcomes, patient experience, and throughput across ambulatory clinics, L&D, and perioperative settings.

The APN & Patient Navigator is responsible for the following overarching areas:

Duties listed under each area reflect the combined clinical (APN) and navigation (PN) functions aligned to Generalist Ob/Gyn practice,

Clinical Assessment & Management (APN)

  • Obstetric and gynecologic histories and physicals; formulation of differential diagnoses; development and implementation of care plans consistent with evidence-based guidelines and patient preferences
  • Ordering, interpreting, and acting on laboratory tests, imaging, and diagnostics; arranging prior authorizations when needed
  • Performance of office-based procedures within training/privileging (e,g, endometrial biopsy, colposcopy, cervical/vulvar/vaginal biopsies; limited obstetric/gynecologic ultrasound interpretation as credentialed); peri-procedural counseling and consent
  • Prescription management, medication reconciliation, and monitoring for safety and efficacy within New Jersey APN scope and collaborative agreements
  • Triage of clinical concerns (phone/portal/in-basket) and timely escalation for urgent evaluation (same-day clinic or ED/L&D)
  • Documentation in the electronic health record (Epic) meeting regulatory, billing, and quality standards

Care Coordination & Navigation (PN)

  • Coordinate appointments, testing, referrals (e.g. MFM, Radiology, Gyn Oncology, Urogynecology), and transitions of care across ambulatory, inpatient, perioperative, and community settings to ensure timely, seamless care
  • Identify and address barriers to care and social determinants of health (e.g. transportation, childcare, financial/insurance challenges, language), connecting patients to community and health-system resources
  • Facilitate prior authorizations and insurance navigation for imaging, procedures, medications, and DME; clarify benefits and out-of-pocket expectations as appropriate
  • Track key care milestones (e.g. prenatal labs and ultrasounds, cervical cancer screening follow-up, post-op checks) and close care gaps through proactive outreach
  • Support transitions (e.g. hospital discharge to clinic follow-up, referral completion) and handoffs between providers/services; ensure key information is communicated and documented
  • Serve as primary liaison for patients and families, fostering trust, engagement, and shared decision-making

Patient Education & Health Literacy

  • Provide understandable education about diagnoses, treatment options, procedures, prenatal care pathways, and postoperative expectations; reinforce teaching across encounters
  • Use teach-back methods and culturally responsive communication; provide translated materials or interpreter services as needed
  • Connect patients to classes, support groups, digital education, and reputable resources

Access, Throughput & Clinic Operations

  • Partner with physicians/APPs and administrative teams to optimize templates, reduce no-shows/cancellations, and improve cycle time and access for new and established patients
  • Perform proactive pre-visit planning; ensure labs, imaging, and external records are available at time of visit
  • Assist with care team inbox management, in-basket routing, and standardized triage protocols to support timely responses

Quality, Safety & Compliance

  • Participate in quality improvement projects (e.g. prenatal care bundles, cervical cancer screening adherence, postpartum visit completion) and track outcomes
  • Ensure compliance with HIPAA, Joint Commission, CMS, and Rutgers/RWJMS policies; report safety events and near-misses; follow infection prevention standards
  • Support clinical trials and research activities as appropriate (e.g. screening, patient education, coordination with research staff)

Interdisciplinary Collaboration & Teaching

  • Collaborate with attending physicians, residents, fellows, nurses, social workers, schedulers, financial counselors, and community partners to deliver team-based care
  • Precept or mentor trainees (e.g. residents, nursing students) within scope; contribute to in-services and case reviews
  • Participate in huddles, case conferences, and L&D/OR coordination as needed
