GENERAL JOB DESCRIPTION
Works continually to assure deliverance of quality clinical care in a safe, efficient, competent, and
effective manner. Prepare records and reports as required in support of services rendered in accordance
with established procedures. Attend and participate in patient care reports, patient care conferences,
team conferences, professional staff conferences and other appropriate professional activities only to
the extent that such attendance and participation is relative to his/her assigned cases and/or
performance of services as determined by the supervisor.
DUTIES & RESPONSIBILITES
• Provides comprehensive medical services and examines patients, using medical instruments
and equipment.
• Elicits and records information about patient’s medical history.
• Orders or executes various tests, analyses, and diagnostic images to provide information on
patient’s condition.
• Analyzes reports and findings of tests and examination, and diagnoses condition of patient.
• Administers or prescribes treatments and medications.
• Promotes health by advising patients concerning diet, hygiene, and methods for prevention of
disease.
• Inoculates and vaccinates patients to immunize patients from communicable diseases.
• Refers patients to medical specialist for consultant services when necessary for patient’s well-
being.
• Review quality reports, assess care gaps and adjust protocol to improve patient’s health.
PHYSICAL DEMANDS
Lifting, twisting, bending and prolonged sitting or standing may be required. The physical movements
and the degree of mobility normally associated with the practice of primary care may be required.
Work under stressful conditions as well as extended hours may be required.
WORK ENVIRONMENT
Potential occasional exposure to low dose radiation hazards, hazardous chemicals and body fluids and
waste. Personal protective equipment provided. May be exposed to infectious and contagious diseases.
MINIMUM QUALIFICATIONS
Graduate of an accredited school of nursing
Graduate of an accredited Nurse Practitioner Program
Current unrestricted Advanced Practice Registered Nurse (APRN) Licensure
Current Nurse Practitioner certification in specialty area (Family Nurse Practitioner and Psychiatric Nurse Practitioner dual certification required)
Current provider card in Basic Life Support (BLS)
Federal DEA Registration
Current CPR certification
Excellent interpersonal skills as well as written and verbal communication skills required.
Medicaid provider number obtainable.
Requires a comprehensive knowledge of adolescent primary care in outpatient settings.
PREFERRED QUALIFICATIONS
Minimum of two years clinical experience in a primary care setting.
Knowledge of the principles and practice of modern medical as related to public health organizations
and community health programs as well as current social and economic problems pertaining to public
health and their impact on primary health care.
