LOCATION: 6580 W. Indian School Rd, Phoenix, AZ, 85033

Work-from-Home Fridays!
You will get to the opportunity to work from home Fridays. This role requires telehealth and in-person appointments in the office or field. CLDF is committed to providing field-based services, meeting clients where they may be most comfortable.

ABOUT CPLC CENTRO DE LA FAMILIA (CDLF)
CDLF provides outpatient behavioral health services for families and individuals of all ages. The program offers counseling and therapy services to those living with serious mental illness, general mental health issues, and substance abuse issues. These issues can be related to depression, anxiety, relationships, loss, substance abuse, suicidal thoughts, and trauma from sexual, emotional and/or physical abuse. Outpatient services offer flexible office hours and home-based services for children and their families, public transit accessibility, therapy in English and Spanish, and professional staff who utilize best practice models for treatment.

HOW WILL YOU MAKE AN IMPACT?
You will facilitate the strategic planning and Child and Family Team (CFT) process for assigned youth and families. This role engages families, stakeholders, formal/ informal supports and individuals involved in the CFT on behalf of the youth and family for the primary purpose of developing a strengths-based plan which meets the youth and family’s needs.

  • Conduct comprehensive intake assessments and collaborate with clients, families, and care teams to develop individualized service plans that address unique needs and strengths.
  • Coordinate and monitor medical, behavioral, and community-based services to ensure seamless continuity of care and positive client outcomes.
  • Provide advocacy, referrals, and skill-building support to empower clients in achieving independence, while maintaining accurate documentation in compliance with agency and state standards.
  • Travel within the community to complete home visits, deliver essential resources, and facilitate case management activities that strengthen client support systems.

WHAT MAKES YOU A GREAT CASE MANAGER?

  • You're experienced in care coordination and strengths-based planning, with knowledge of community-based behavioral health services and the ability to engage youth, families, and multidisciplinary teams effectively.
  • You're highly organized, culturally aware, and trauma-informed. You're skilled in conducting assessments, developing individualized service plans, and maintaining compliance with documentation standards while addressing barriers to care.
  • You're able and ready to go into the community to meet the needs of your clients.

MINIMUM QUALIFICATIONS

    Meets Behavioral Health Technician criteria as defined in Office of Behavioral Health Licensing rules per the Arizona Administrative Code R9-20-204 and has:
      Bachelor’s degree and at least one year of full-time behavioral health work experience, or Associate degree and at least two years of full-time behavioral health work experience, or Over 5 years of full-time behavioral health work experience.
    Knowledge of community based behavioral health services preferred. Possess a valid driver’s license and ability to be insured under automobile insurance policy. Ability to obtain fingerprint clearance card, CPR and First Aid.

Other Requirements

    Ability to work a flexible schedule as needed per program requirements. Basic typing/computer skills. Demonstrated verbal and written communication skills. Ability to maintain positive relationships with consumers, community partners and team. Demonstrated organization and time management skills and able to manage workload with multiple deadlines and competing demands. Ability to attend required Child and Family Team training within one month of assuming position and participate in ongoing learning requirements.
