ABOUT CPLC CENTRO DE LA FAMILIA
CPLC Centro De La Familia
provides outpatient behavioral health services for individuals and families of all ages. Our program offers counseling and therapy services for those living with serious mental illness, general mental health challenges, and substance use concerns. Services address issues such as depression, anxiety, relationship challenges, loss, trauma, substance abuse, and suicidal ideation.

Outpatient services include flexible office hours, home-based services for children and families, access via public transit, therapy in English and Spanish, and professional staff who utilize evidence-based treatment models.

OBJECTIVE AND POSITION SUMMARY
The High Needs Case Manager Lead (HNCM Lead) supervises HNCM staff and is responsible for clinical coordination of services, providing direct and indirect care. This role ensures high-quality, client-centered care, compliance with organization and regulatory standards, and collaboration with community partners.

SUPERVISORY RESPONSIBILITIES

  • Supervise and support a team of case managers
  • Provide coaching, training and regular performance feedback
  • Conduct regular team meetings to ensure communication of updates, challenges and program goals
  • Assist Clinical Director in clinical training and staff development
  • Ensure that performance reviews are completed annually for staff under direct supervision, including establishing specific individual and professional development goals
  • Provide recommendations and implement employee disciplinary actions to include termination when necessary
  • Assist in interviews, select, and make recommendations on hiring new staff members
  • Participate in monitoring data collection for direct services as assigned, including intakes and discharges, caseload capacity, clinical documentation, covered service encounters and productivity, appointment scheduling, re-engagement, scheduling of service and outcome related measures
  • Ensure billing and production expectations are met by all staff
  • Participate in internal and external committees and management of team meetings
  • Attend various community meetings as assigned and report relevant information to the Clinical Director
  • Be available to offer leadership and cooperation to members and community providers in the development and application of best practices

SPECIALIZATIONS

  • Knowledge of community mental health systems and recovery-oriented care

PRIMARY DUTIES AND ESSENTIAL FUNCTIONS

  • Monitor daily operations to ensure effective and timely service delivery
  • Coordinate staff schedules and caseload assignments to ensure coverage and efficiency
  • Implement and uphold agency policies and procedures
  • Ensure member assessments and treatment plans are being followed and services are documented properly
  • Seek consultation when knowledge, skill and/or ability may exceed scope of practice
  • Provide guidance on complex client situations, including crisis intervention
  • Ensure referrals to appropriate community resources
  • Ensure compliance with HIPAA, AHCCCS, state behavioral health regulations and organizational policies
  • Monitor documentation for timeliness, accuracy and completeness
  • Prepare of internal and external audits, responding to findings and implementing corrective actions
  • Act as a liaison with other departments, agencies and community partners
  • Attend inter-agency and multidisciplinary team meetings as needed
  • Represent the clinic in community forums and stakeholder meetings
  • To remain informed of changes in contract requirements, AHCCCS requirements and Arizona state statutes and rules related to all populations being served by the program. Ensure compliance with all changes
  • Assist with maintaining documentation, including program manuals and clinical guidelines and procedures
  • Coordinate community outreach and advocacy efforts related to mental health and wellness
  • Develop and implement evidence-based practices, ensuring trauma-informed and recovery-oriented approaches
  • Coordinate with other service providers to ensure continuity of care
  • Provide regular reporting to the Clinical Director as assigned
  • Perform other duties as assigned

MINIMUM QUALIFICATIONS AND COMPETENCIES

Education/Background:

  • Bachelor’s degree in Social Work, Psychology, Public Health or related field required (Master’s degree preferred)
  • Minimum of 3 years of experience in behavioral health

Additional Competencies:

  • Strong leadership and interpersonal skills
  • Ability to organize, prioritize, and manage multiple responsibilities
  • Proficient in Microsoft 365 and electronic health records (EHR) systems
  • Strong written and verbal communication skills
  • Culturally competent with ability to work with diverse populations
