Care Coordinator at Thibodaux Regional Health System – Thibodaux, Louisiana

The Care Coordinator executes processes and procedures that establish and promote compliance, satisfaction, quality and performance standards. This position interacts with patients meeting criteria for care coordination, provides education and other resources to optimize the patient’s health and wellness across the care continuum.

Educational and/or Professional Qualifications:

• Associates or Bachelor degree preferred

Previous Experience

• Previous experience interfacing with the public
• Ability to prioritize responsibilities and use appropriate time management skills.

Skills

• Ability to work collaboratively across the care continuum, including ambulatory care providers, home care, community services, and other post-acute services
• Strong interpersonal, oral and written communication skills: effective interpersonal and problem-solving skills
• Proficiency with Microsoft Office Suite, spreadsheet expertise and other computer software programs
