Your Role as a Home Health Licensed Clinical Social Worker
As a Licensed Clinical Social Worker (LCSW), you will provide psychosocial support and behavioral health counseling to patients receiving care in their homes. You will play a vital role in addressing social, emotional, and environmental barriers that impact health outcomes.
Responsibilities include:
- Provide psychosocial assessments, counseling, and behavioral health supportDevelop and coordinate care plans to address social, emotional, and environmental needsConnect patients with community resources and support servicesAssist patients and families in navigating social, financial, and healthcare challengesCollaborate with physicians and interdisciplinary care teams to support patient outcomesEducate patients and caregivers on coping strategies and available resourcesMaintain accurate and timely documentation in accordance with Medicare guidelinesDocument visits efficiently using AI-assisted EMR tools

Qualifications
- Active, unencumbered Florida Licensed Clinical Social Worker (LCSW) licenseMinimum two years of clinical counseling experienceHome health or telehealth experience preferredValid driver’s license, reliable transportation, and auto insurance
