The Licensed Practical Nurse (LPN) serves as a clinical leader responsible for overseeing resident wellness, coordinating care, administering medications, monitoring changes in condition, and supporting caregivers to ensure exceptional resident outcomes.

This role partners closely with the Director of Nursing, Executive Director, physicians, pharmacies, families, and community team members to deliver safe, compassionate, person-centered care while maintaining compliance with all applicable regulations and company standards.

Job Description

Resident Care & Clinical Support

• Provide direct nursing care in accordance with physician orders, resident service plans, and state regulations.

• Administer medications and treatments safely and accurately.

• Monitor residents for changes in condition and report concerns promptly.

• Complete nursing assessments and ongoing wellness observations.

• Collaborate with physicians, pharmacies, home health agencies, hospice providers, and outside healthcare professionals.

• Respond appropriately to resident emergencies and urgent clinical situations.

• Promote resident dignity, independence, and quality of life.

Medication Management

• Administer medications following all company policies and medication administration standards.

• Monitor medication effectiveness and potential adverse reactions.

• Ensure medication carts, storage areas, and controlled substances remain secure and compliant.

• Communicate medication changes promptly with appropriate team members.

• Assist with pharmacy coordination, medication deliveries, and medication documentation.

Documentation & Compliance

• Complete accurate and timely clinical documentation.

• Maintain resident records according to company policy and state regulations.

• Document resident assessments, incidents, physician communications, and changes in condition.

• Assist with survey readiness activities.

• Ensure confidentiality of resident information.

Team Leadership & Support

• Serve as a clinical resource for caregivers, QMAs, and other nursing staff.

• Provide coaching and guidance regarding resident care and safety.

• Assist with training new nursing team members.

• Communicate effectively during shift change and interdisciplinary meetings.

• Promote teamwork, accountability, and professionalism.

Resident & Family Experience

• Build trusting relationships with residents and families.

• Communicate resident updates professionally and compassionately.

• Respond promptly to resident and family concerns.

• Encourage resident choice while promoting health and safety.

• Help create an environment where residents feel respected, valued, and cared for.

Requirements:
  • Current Indiana Licensed Practical Nurse (LPN) license in good standing.
  • Current CPR certification or ability to obtain upon hire.
  • Minimum one year of nursing experience.
  • Strong clinical assessment and communication skills.
  • Ability to work independently while collaborating with interdisciplinary teams.
  • Basic computer proficiency and experience with electronic health records.

Preferred Qualifications

  • Experience in assisted living, senior living, long-term care, or healthcare.
  • Medication administration experience.
  • Supervisory or charge nurse experience.
  • Experience with resident assessments and care planning.

Physical Requirements

  • Ability to stand and walk for extended periods.
  • Ability to assist residents with mobility as needed.
  • Ability to lift, push, or transfer up to 50 pounds with or without reasonable accommodation.
  • Ability to respond quickly during emergencies.
