Citizens Memorial Hospital (CMH) is hiring a Rural Health Transformation Program (RHTP) Manager to join our Quality Department in Bolivar, Missouri— someone who leads with skill, compassion, and a drive to positively impact every patient and family we serve.

What Your Day Looks Like: In this role you will lead local community Hub operations and regional partnership initiatives that improve population health across a 5-county region. This role coordinates healthcare and community partners, strengthens regional care coordination and referral networks, monitors program performance, and advances innovative, community-based strategies that improve health outcomes and support sustainable rural healthcare transformation.
As a Rural Health Transformation Program Manager, you will be:

Leading local community Hub operations and RHTP implementation.

Building partnerships with healthcare providers, community organizations, and regional stakeholders.

Facilitating governance activities and stakeholder engagement.

Strengthening care coordination, referral pathways, and community service networks.

Monitoring performance, managing reporting requirements, and driving continuous improvement.

Supporting innovative, value-based care initiatives that improve rural health outcomes.
Qualifications:
Education: Bachelor’s degree in public health, health administration, business, social work, or related field required, Master's Degree preferred
Experience: At least one year experience in community health, business operations, and/or program management
Skills: Ideal candidates are strong communicators and project leaders who excel at collaboration, execution, relationship building, and advancing community health initiatives.
Licensure/Certification:
- CPI training at appropriate level within one year, renew annually
- HazMat Awareness (Extended) within 3 months, renewed annually
Physical Requirements:
- Light mobility: occasional walking, standing, and short-distance travel to clinics or community partner sites.
- Mostly sedentary work: long periods of sitting for computer tasks, meetings, and reporting.
