Wage Range: $28.44 - $45.51 per hour

Job Summary

Serves as a patient advocate to help patients and family members navigate the complex healthcare system. Provides information about community resources, makes referrals, and assists with accessing services. Assists patients with completing applications for health insurance, transportation, disability, and various other basic needs. Functions as a liaison between healthcare providers and patients. May provide case management for specific patients. While the office is based out of the EvergreenHealth Kirkland hospital, much of the work will involve patients currently admitted, patients in the Emergency Department, walk in appointments, patients from primary/specialty are clinics, and residents in our surrounding area. Responsible for conducting outreach activities to assist residents in the community served by EvergreenHealth gain access to healthcare and health related services.

Primary Duties:

Screens and responds to incoming requests: Provides information about community resources, referrals, and assistance with accessing services; Screens requests for service; assesses to make initial determination of appropriate type and level of service needed; Collaborates with the care team.
Conducts initial assessments with clients and develops a plan for service or referral.
Collaborates with various team members
Provides office-based case management per guidelines for programs. Refers and links patients to primary care and/or specialty care as appropriate.
In collaboration with department management, identifies and conducts outreach activities within the community.
Performs other duties as assigned.

License, Certification, Education or Experience:

REQUIREDfor the position:

Bachelors Degree
Age-specific or behavior-specific knowledge and experience in working with specific patient populations (i.e., children, elderly, those with emotional / behavioral dysfunction) relevant to area of assignment.
Verbal and written language skills to effectively communicate, ensure, and provide for patient needs, and to provide a legal record of this care.
Accountability for professional practice, commitment to patient care, and commitment to uphold the Patient Bill of Rights.

DESIREDfor the position:

Bachelor's Degree in Social Work or other related field
Background in social work or psychology
1-2 years of experience in a healthcare agency or hospital
Fluent in Spanish
Excellent written language skills in Spanish
