Nurse Practitioner (Full or Part Time) – Outpatient Palliative Care (flexible location - either North, South or Central Berkshire County location)

Berkshire Health Systems is seeking a compassionate and skilled Nurse Practitioner (NP) to join our Outpatient Palliative Care team serving patients across Berkshire County. This is an exceptional opportunity for an NP who is passionate about improving quality of life for individuals living with serious illness and working collaboratively with specialty practices and primary care teams to deliver coordinated patient-centered care.
About the Role
- Provide outpatient palliative care consultation and longitudinal management for patients with serious illnesses, particularly in pulmonary, cardiology, and complex chronic conditions. Office practice location can be located in either North Adams, Great Barrington and Pittsfield with some travel between practice sites.
- Manage symptoms such as pain, dyspnea, fatigue, nausea, anxiety, and depression that affect quality of life and function.
- Facilitate goals-of-care discussions, advance care planning, and shared decision-making with patients and their families.
- Collaborate closely with specialty and primary care providers to align treatment plans with patient values and clinical goals.
- Support transitions of care and help reduce avoidable hospital utilization through proactive, coordinated management.
- Assist with building a palliative medicine program in collaboration with colleagues.

Qualifications
- Master’s degree in Nursing and completion of an accredited Nurse Practitioner program.
- Current Massachusetts NP licensure (or eligibility).
- Experience in palliative care and palliative medicine certification preferred.
- Excellent communication skills, with demonstrated ability to lead sensitive conversations and build trust with patients and families living a serious illness.
- Strong clinical judgment and commitment to interdisciplinary teamwork.
