The Clinical Navigator is a Registered Nurse with experience in oncology services who navigates patients throughout the process of the continuum of cancer care from diagnosis, treatment, to survivorship in collaboration with the multidisciplinary team, and according to agreed clinical guidelines, with the objective of coordinating high quality patient care across all care delivery sites. Using the nursing process, provides education and resources to facilitate informed decision-making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum.
