East Arkansas Family Health Center is seeking a compassionate and organized Behavioral Health Care Manager to join our integrated Behavioral Health team. This position plays a key role in our Collaborative Care Model (CoCM) by coordinating behavioral and physical healthcare services for patients experiencing mental health and substance use disorders.
As a core member of the interdisciplinary healthcare team, you will collaborate with primary care providers, psychiatric consultants, behavioral health professionals, and community partners to ensure patients receive comprehensive, evidence-based, patient-centered care.

- Coordinate behavioral health and primary care services for an assigned patient caseload.
- Screen and assess patients for common mental health and substance use disorders.
- Educate patients and families regarding behavioral health conditions and available treatment options.
- Monitor patient progress, treatment response, and medication adherence through in-person and telephone follow-up.
- Provide brief evidence-based behavioral interventions, including Motivational Interviewing, Behavioral Activation, and Problem-Solving Therapy.
- Collaborate with primary care providers and psychiatric consultants to develop and implement individualized treatment plans.
- Participate in weekly Collaborative Care case review meetings with the psychiatric consultant.
- Maintain patient registries and track clinical outcomes to ensure timely follow-up and quality care.
- Coordinate referrals to specialty behavioral health services, substance use treatment programs, community resources, and social service agencies.
- Assist eligible patients with enrollment in East Arkansas Family Health Center's Patient Assistance Program.
- Connect patients with community resources including housing assistance, transportation, financial assistance, vocational rehabilitation, and other supportive services.
- Develop relapse prevention and self-management plans for patients successfully completing treatment.
- Maintain accurate documentation in the Electronic Health Record (EHR) and other required systems.
- Participate in community outreach activities, quality improvement initiatives, and program development.
- Prepare monthly reports and other program documentation as required.
- Maintain compliance with HIPAA, OSHA, and all applicable federal and state regulations.
- Perform other duties as assigned.
- Current Arkansas licensure in one of the following:
- Licensed Master Social Worker (LMSW)
- Licensed Professional Counselor (LPC)
- Licensed Marriage and Family Therapist (LMFT)
- Licensed Social Worker (LSW)
- Registered Nurse (RN) (BSN preferred)
- Nurse Practitioner (NP)
- Licensed Psychologist
- Experience in behavioral health, care management, case management, counseling, nursing, or a related healthcare field.
- Knowledge of behavioral health assessment, treatment planning, and care coordination.
- Experience with evidence-based behavioral interventions such as Motivational Interviewing, Behavioral Activation, or Problem-Solving Therapy.
- Strong communication, organizational, and interpersonal skills.
- Ability to collaborate effectively within an interdisciplinary healthcare team.
- Experience working in a Federally Qualified Health Center (FQHC) or community health center.
- Experience working within a Collaborative Care Model (CoCM).
- Experience with Electronic Health Record (EHR) systems.
- Experience coordinating behavioral health and primary care services.
- Bilingual skills are a plus.
