Compensation: $130,000 - $135,000 annually
Schedule: Monday–Friday | 8:30 a.m.– 5:00 p.m., with rotating on-call responsibilities

The YoloCares Clinical Team
You will join a highly skilled, compassionate, interdisciplinary clinical team dedicated to providing patient- and family-centered hospice care. YoloCares’ clinical team includes registered nurses, hospice aides, social workers, spiritual care counselors, nurse practitioners, and physicians who work collaboratively to support patients and families through every stage of the hospice journey. The team is grounded in clinical excellence, mutual respect, and thoughtful coordination, with a shared commitment to dignity, comfort, and individualized care.
About the role
As Clinical Manager, you will lead an interdisciplinary team in delivering exceptional, patient-centered hospice care. In partnership with the Chief Clinical Officer, you will oversee daily clinical operations while coaching and developing clinical staff to build a collaborative, high-performing care team. As a member of the clinical leadership team, you will also participate in a shared administrative after-hours on-call rotation, providing leadership support outside of regular business hours.
Key Responsibilities
Clinical Leadership & Team Development
- Provide coaching, mentorship, and performance feedback to clinical team members.
- Support staff development through orientation, education, and ongoing competency building.
- Conduct regular ride-along visits with clinical staff to reinforce best practices, evaluate performance, and promote consistent standards of care.
- Foster a culture of collaboration, accountability, and continuous improvement.
Patient Care & Clinical Excellence
- Provide clinical guidance for complex patient situations, symptom management, and hospice care planning.
- Ensure appropriate hospice eligibility practices using Medicare guidelines, Local Coverage Determinations (LCDs), and Palliative Performance Scale (PPS) criteria.
- Support timely, coordinated care delivery for patients and families.
- Promote evidence-based hospice and palliative care practices.
Operations & Resource Management
- Oversee staffing, scheduling, and workload distribution to ensure patient care needs are met.
- Monitor census, staffing needs, and clinical resources to support efficient operations.
- Assist with patient care coverage and RN case management responsibilities as needed.
- Maintain appropriate utilization of hospice resources.
Quality, Compliance & Improvement
- Ensure clinical documentation meets regulatory, accreditation, and organizational standards.
- Support compliance with Medicare Hospice Conditions of Participation, Joint Commission standards, and California hospice regulations.
- Participate in Quality Assurance and Performance Improvement (QAPI) activities.
- Identify opportunities to improve clinical workflows, communication, and patient outcomes.
Communication & Collaboration
- Facilitate effective communication among interdisciplinary team members, physicians, patients, families, and community partners.
- Support problem-solving and decision-making in complex clinical situations.
- Promote a respectful, inclusive environment that honors diverse beliefs, values, and patient preferences.
- Current California Registered Nurse (RN) license in good standing.
- Bachelor of Science in Nursing (BSN) from an accredited nursing program.
- Minimum of three (3) years of nursing experience, including at least one (1) year in hospice, home health, palliative care, or a related setting.
- At least one (1) year of healthcare leadership or supervisory experience.
- Strong clinical knowledge of hospice and palliative care, including pain and symptom management and end-of-life care.
- Experience supporting interdisciplinary care teams and collaborating with healthcare professionals.
- Excellent communication, coaching, and problem-solving skills.
- Ability to prioritize competing clinical and operational needs in a fast-paced environment.
- Ability to meet YoloCares employment requirements, including background screening.
- Ability to perform the essential functions of the position, with or without reasonable accommodation, including lifting up to 60 pounds.
Preferred Qualifications
- Previous hospice leadership or clinical management experience.
- Knowledge of Medicare Hospice Conditions of Participation, Joint Commission standards, California hospice regulations, and payer requirements.
- Experience with Quality Assurance and Performance Improvement (QAPI) initiatives.
- Spanish language skills.
