Home Care Assistant / Certified Nursing Assistant

Reports To: Home Care Director

What You'll Do

As a Home Care Assistant / Universal Aide, you will:

  • Provide personal care and assistance with activities of daily living, including bathing, dressing, grooming, oral care, toileting, and meal support.
  • Deliver home care services in accordance with individualized care plans and physician-directed services.
  • Observe and report changes in participants' physical, emotional, or behavioral status to the clinical team.
  • Assist with restorative and supportive care activities that promote independence and well-being.
  • Facilitate and participate in group and individual activities designed to enhance engagement and quality of life.
  • Support participants during meals and encourage proper nutrition and hydration.
  • Accompany participants to medical and community appointments when needed.
  • Assist with transportation activities, including participant loading, unloading, and escorting.
  • Maintain accurate documentation and participant records.
  • Contribute to a safe, clean, and welcoming environment.
  • Collaborate with nurses, therapists, caregivers, and other interdisciplinary team members to deliver exceptional participant-centered care.
What We're Looking ForRequired Qualifications
  • High school diploma or equivalent.
  • Current North Carolina Certified Nursing Assistant (CNA) certification.
  • Current CPR certification.
  • Minimum of one year of experience working with older adults, seniors, or frail populations.
  • Valid driver's license, reliable transportation, and proof of insurance.
  • Ability to lift up to 40 pounds and perform the physical demands associated with direct care.
