Southern Highlands Community Mental Health Center is seeking a compassionate, organized, and motivated Case Manager/Care Coordinator II – School-Based to join our growing behavioral health team. This position is embedded within a local school and provides direct support to students and families by coordinating behavioral health, educational, medical, and community services.

Position SummaryAs a School-Based Case Manager/Care Coordinator, you will work collaboratively with students, families, school personnel, healthcare providers, and community partners to ensure students receive the support and resources they need to thrive. You will develop person-centered treatment plans, coordinate services, monitor student progress, and help remove barriers to care while promoting emotional wellness and positive outcomes.
What You'll Do- Coordinate behavioral health, medical, educational, and community services for students
- Conduct person-centered assessments and assist with treatment planning
- Provide care coordination and case management services
- Facilitate individual and group Social Emotional Learning (SEL) sessions using evidence-based curricula
- Serve as a liaison between students, families, schools, and community providers
- Support students experiencing behavioral health crises
- Participate in IEP, 504, and other school meetings as appropriate
- Connect families with community resources including housing, food assistance, transportation, and other supports
- Maintain timely documentation within the electronic health record (EHR)
- Participate in school-wide behavioral health education and outreach activities
- Bachelor's or Master's degree in Social Work, Counseling, Psychology, Nursing, Sociology, Teacher Education, Behavioral Health, Criminal Justice, Political Science, Recreational Therapy, or another qualifying Liberal Arts field approved by the WV Board of Social Work
- Valid driver's license with a good driving record
- Ability to work flexible hours as needed, including occasional evenings or school events
- Strong communication, organizational, and relationship-building skills
- Experience working in a school setting or with children and adolescents
- Experience in behavioral health or community mental health
- Knowledge of Certified Community Behavioral Health Clinic (CCBHC) services
- Current professional license (LSW, RN, or similar), if applicable
- Demonstrated compassion, advocacy, and commitment to helping youth succeed
