Position Summary

The Community Health Worker (“CHW”) is a member of WCC's Community Public Health Team (CPHT) responsible for connecting patients to clinical care, community resources, and social support services. Working closely with the care team and community partners, the CHW conducts outreach, follows up on referrals, screens for social needs, and helps patients navigate the health care system.

Compensation and Benefits

The compensation for this position is $22.00 hourly. Education and Experience

· High school diploma or GED required; associate's or bachelor's degree in public health, social work, human services, or a related field preferred

· CHW certification preferred; candidates actively enrolled in a certification program will be considered

· Experience in a community health, patient navigation, care coordination, or similar role serving underserved populations; experience with Medi-Cal or safety-net programs strongly preferred

· Experience conducting outreach or community education in settings such as health fairs, schools, faith-based organizations, or community groups

Essential Position Responsibilities

· Follow up with patients after referral initiation to confirm understanding, intent, and barriers; track referrals from initiation through completion in Epic

· Contact patients who have not completed referred services to address barriers, provide reminders, and re-engage them in care

· Administer SDOH screening tools to identify unmet social needs; connect patients with appropriate community resources and government benefit programs including Medi-Cal, CalFresh, Covered California, and CHIP

· Build and maintain current knowledge of community resources, eligibility criteria, and referral processes; develop relationships with CBOs and partner agencies

· Refer patients to enrollment specialists for government program applications and renewals

· Assist patients with scheduling across WCC service lines: primary care, dental, behavioral health, and outreach and enrollment, with attention to those who face barriers to self-scheduling

· Outreach to patients with pending or overdue care gaps to encourage scheduling and facilitate access

· Coordinate care for patients with complex needs across multiple WCC service lines or external specialty providers

· Represent WCC at community health fairs, outreach events, school-based events, and faith-based gatherings

· Conduct outreach targeting priority populations including Medi-Cal enrollees, uninsured individuals, families with children, and Spanish-speaking community members

· Serve as a cultural and linguistic liaison between patients and the WCC care team, supporting health system navigation

· Document all patient contacts, referral follow-ups, resource linkages, and care coordination activities in Epic accurately and timely

· Complete required compliance training through Relias; maintain current knowledge through ongoing professional development

· Complete other duties as assigned in support of WCC's mission

Skills

· Strong interpersonal and relationship-building skills with the ability to engage compassionately with patients from diverse cultural, linguistic, and socioeconomic backgrounds

· Excellent verbal and written communication skills; able to explain complex health, insurance, and resource information clearly to patients and families

· Highly organized with the ability to manage a caseload, multitask, and maintain accurate records

· Self-directed, resourceful, and adaptable; exercises good judgment and creative problem-solving

· Able to work collaboratively with diverse populations and interdisciplinary teams

· Proficient in Microsoft Office Suite; familiarity with EHR systems (EPIC)

· Knowledge of community resources, social service programs, and eligibility requirements in the Los Angeles Harbor/South Bay area, or ability to quickly learn

· Bilingual English/Spanish required
