The Field Case Manager is responsible for managing comprehensive patient care plans, coordinating healthcare services, and supporting patients in achieving their health goals. The role requires strong clinical judgment, organization, and collaboration skills to ensure effective care delivery.
- Conduct patient assessments to identify healthcare needs, develop individualized care plans, and establish achievable goals.
- Coordinate access to healthcare services, including specialist referrals, therapies, and community support resources.
- Monitor patient progress, evaluate care plan effectiveness, and adjust strategies when needed.
- Educate patients and families on disease management, medication adherence, and healthy lifestyle practices.
- Act as a key liaison between patients, healthcare providers, insurance partners, and community resources to facilitate coordinated care.
- Maintain accurate documentation of patient interactions, care plans, and outcomes using electronic health record systems.
- Ensure compliance with healthcare regulations, organizational policies, and ethical standards.
- Participate in team meetings, case discussions, and professional development activities.
- Travel within the assigned region to conduct home visits and attend patient appointments.
The ideal candidate is an experienced healthcare professional with strong case management capabilities, excellent communication skills, and the ability to independently manage complex patient needs.
- Bachelor’s degree in Nursing (BSN) or a related healthcare field required; a Master’s degree is preferred.
- Current and unrestricted Registered Nurse (RN) license in the province of employment.
- 3–5 years of clinical experience, preferably in case management, home healthcare, or a related healthcare environment.
- Strong understanding of healthcare systems, insurance processes, and available community resources.
- Excellent assessment, critical thinking, care planning, and problem-solving skills.
- Strong interpersonal, communication, and negotiation abilities when working with patients and healthcare stakeholders.
- Experience using electronic health records (EHR) and healthcare-related software platforms.
- Ability to manage multiple cases independently while maintaining attention to detail and quality standards.
- Valid driver’s license and reliable transportation required for field-based responsibilities.
- Bilingual English/French skills are considered an asset.
