This position is fully remote, but must live near a branch office.

Your Role as a Chronic Care Coordinator:

Qualifications:

Licensed Practical Nurse (LPN) required
• Minimum 1-year experience in homecare, Skilled Nursing Facility or Primary Care setting
• Minimum 1-year experience in Chronic Care Management preferred
• Knowledge of medical terminology and disease specific interventions
• Computer/EMR system savvy
