Home Health Clinical Manager (RN) - $10,000 Sign on Bonus in Las Cruces, New Mexico at Sierra Home Health and Hospice

About the Role

Sierra Health Care is looking for a Clinical Manager to lead home health clinical operations in Las Cruces. In this role, you'll oversee a team of nurses and therapists, guide individualized patient care planning, and make sure every patient receives safe, coordinated, and compassionate care. Key Responsibilities

• Supervise and support nursing and clinical staff to ensure safe, patient-centered care delivery
• Recruit, orient, and evaluate clinical staff, including ongoing competency validation
• Oversee individualized patient care plans and lead care conferences
• Coordinate closely with physicians, community partners, and the interdisciplinary team
• Lead quality and compliance efforts through QAPI, infection prevention, and emergency preparedness
• Ensure timely, accurate clinical documentation in the EMR
• Stay available for consultation and on-call coverage to support continuity of care
• Manage clinical staffing, scheduling, and resources to keep operations running smoothly

What We're Looking For

• Graduate of an accredited nursing or therapy program; current, unrestricted New Mexico license required
• Minimum 3–5 years of clinical experience, including at least 1 year in home health or a related setting
• Demonstrated ability to supervise clinical staff, coordinate care, and maintain regulatory compliance
• Strong knowledge of Medicare and Medicaid Conditions of Participation and state licensure standards
• Proven leadership, communication, and organizational skills
• EMR proficiency and comfort managing multiple clinical priorities
• Valid driver's license and reliable transportation
