Summary:

The ACT Lead Psychiatric Nurse is a member of the Assertive Community Treatment (ACT) team, responsible for overseeing and ensuring efficient nursing related care for members with mental illness. The ACT Lead Psychiatric Nurse is directly responsible for, and under the immediate supervision of the Director of Crisis Services and is expected to collaborate with, and work alongside, ACT Assistant Director.

Essential Responsibilities, include but are not limited to:

  • Complete annual nursing evaluations for clients with medically complex needs, including record reviews, client interviews, and coordination with collateral contacts.
  • Monitor and coordinate client medical appointments, ensuring all required exams and follow-up care are compliant with state regulations.
  • Administer injectable psychotropic medications as ordered by the psychiatrist.
  • Review and communicate lab results to the psychiatrist and/or medical doctor, and ensure follow-up as indicated.
  • Take vital signs, prepare Physician Order Sheets, and organize lab work for medical visits.
  • Serve as a liaison between pharmacies, external medical providers, and internal teams regarding clients’ medical needs.
  • Manage medication logistics, including communicating needs to the pharmacy, processing medication orders, and securing prior authorizations or supporting appeals when needed.
  • Participate in interdisciplinary team meetings to support client-centered service planning, implementation, and evaluation.
  • Provide clinical input on living arrangements, assistive devices, and other medical resources in collaboration with program staff.
  • Act as Lead Medication Trainer for program staff, delivering onboarding and ongoing training, and ensuring quality documentation and procedures.
  • Provide clinical guidance to other nursing staff across programs by offering support, mentorship, and consultation on medication management, documentation standards, and clinical protocols. Collaborate with program leadership to promote consistency in nursing practices and help ensure adherence to agency and regulatory guidelines. Offer consultation and support to program staff and fellow nurses regarding clinical matters and care coordination.
  • Respond to medical emergencies promptly, completing incident reports within 24 hours.
  • Conduct home visits following hospital discharges or as medically necessary to ensure continuity of care and client well-being.
  • Maintain compliance with Medicaid Rule 132/140 documentation standards and meet assigned productivity targets.

SUPERVISORY RESPONSIBILITIES:

None

CLINICAL EXPECTATIONS:

  • Manage referrals, admissions, and discharges.
  • Provide oversite to staff engaging in discharge planning.
  • Assist staff with managing clients at times of high volume.
  • Will provide and document crisis interventions and additional risk assessments, as needed, and will complete incident reports when appropriate within a 24hr period.
  • Ensure all services provided are in accordance with the client’s individual goals and needs.
  • Enter, maintain, and update client information in Envision Unlimited’s Electronic Health Record (Quantum). i.e. face sheet, treatment plan, and diagnoses.
  • Attending scheduled clinical meetings including group supervisions, clinical and other staff team meetings.
  • Arrive to work, meetings, and all work-related functions on time; Report any absence, tardiness, or schedule change to supervisor as soon as possible. Maintain appropriate attendance record using Paylocity.
  • Other duties as assigned.

Requirements:

QUALIFICATIONS:

  • Must have a BSN, current state of Illinois RN License.
  • Must have at least two years of related experience working with individuals with chronic mental illnesses within the past five years.
  • Able to complete mandatory annual trainings.
  • Must have a valid driver’s license.
  • Must have the ability to walk, bend, stoop, sit, stand, reach, and lift 50 pounds. Must possess a valid DL, as well as pass a background check.
  • Be comfortable working within an agency setting serving clients with MI/DD
