Home Health Coordinator - PDN & Skilled Nursing

We are seeking an experienced and detail-orientated professional with a strong background in Private Duty Nursing (PDN) and Skilled Nursing (SN) to join our home health team.

Pay: 21/HR

Location: 24044 Cinco Village Center Bld., Katy, TX 77494

Qualifications:

Previous experience in home health administration or coordination.
Strong knowledge of Private Duty Nursing and Skilled Nursing operations.
Familiarity with patient referrals, authorizations, scheduling, documentation, and clinical coordination.
Knowledge of Medicaid/insurance processes and regulatory compliance.
Excellent organizational, communication, and multitasking skills.
Ability to work independently in a fast-paced environment.

Experience in both PDN and Skilled nurse is strongly preferred.
