OBJECTIVE AND POSITION SUMMARY

The Peer Case Coordinator is a trusted professional with lived experience who delivers integrated peer support and case management services to Veterans, service members, and their families who are at risk for suicide. This role focuses on empowering participants through individualized planning, goal setting, and connection to behavioral health and community resources. Operating under the SSG Fox Suicide Prevention Grant Program, the Peer Case Coordinator plays a key role in outreach, engagement, and service navigation, helping individuals stabilize and thrive across a continuum of care.

MINIMUM QUALIFICATIONS AND COMPETENCIES

Education/Background​

• Required: Bachelor’s degree in Social Work, Psychology, or related field
• Required: Peer Support Specialist Certification (or willingness to obtain certification within 60 days of hire date)
• Required: At least six months of experience in peer support, case management, social services, or outreach
• Strong knowledge of behavioral health systems, VA services, and community resources
• Excellent interpersonal, communication, and organizational skills
• Ability to work both independently and as part of a team
• Must possess a valid driver’s license and meet insurability requirements
• Proficiency in Microsoft Office and comfort with digital documentation systems
