Job Overview:

The nurse practitioner functions as an independent care provider in collaboration with other healthcare professionals. The care is characterized by comprehensive health assessments and management of patients with acute and/or chronic health problems in the heart and vascular service line (Invasive and Non-Invasive).

Job Qualification

• Licensed Registered Nurse in the state of New Jersey
• Certified Nurse Practitioner in the state of New Jersey
• Masters/Doctorate in Nursing from an accredited program
• ACLS/BLS/DEA/CDS certification
• Must have cardiac and/or critical care experience
• Excellent communication and interpersonal skills

Scheduling Requirement:

• 37.5 Hours per week
• Weekend possible as needed

Essential Functions

• Conducts thorough patient assessments, including medical history and physical examinations and review of diagnostic test results.
• Develops and implements comprehensive care plans for patients undergoing invasive and non-invasive heart and vascular procedures.
• Monitor, review and manage all cardiovascular patients.
• Provides pre and post procedure care management for invasive and non-invasive cardiovascular patients.
• Cardiovascular education and support to patients and their families.
• Facilitates discharges for Heart and Vascular service line patients.
• Provides education to staff and community regarding cardiac conditions and procedures.
• Participates in quality improvement initiatives to enhance patient care and outcomes.
• Collaboration with a multidisciplinary team, including cardiologists, NP’s, nurses and social workers.
