Description

Patient Transfer Coordinator expedites patient transfers from outside hospitals, urgent/immediate care centers and physician practices for direct admission to the facility by coordinating the connection of accurate physicians to maintain an optimized referral within SGHS.

Collaborates with physician, nurses and other medical professionals to ensure timely coordination of transfer.

Assist as needed with the transfer of patients to higher or lower level of care.

Serves as contact person for bed management for any direct admission concerns. Ensure throughput on units and compliance with transfer process.

Develops collaborative relationships with physicians to enhance quality practice. Develops and maintains sound intradepartmental and interdepartmental working relationships with team members, and management. Acts to resolve issues in a timely manner. Investigates unusual occurrences and follows appropriate reporting procedure.

Function as a department team member to support the Transfer Center and hospital goals.

Able to prioritize and perform multiple tasks effectively in a time sensitive environment to achieve optimal outcomes.

Coordinate and collaborate with physicians, all hospital nursing units, transportation, bed control registrar, environmental services and House Manager regarding patient placement.

Communicate with clinical unit nursing staff the new admission information and develop a plan for acceptance of the new admission within 30 minutes.

During high call volume or limited bed capacity, remains calm and provides solutions effectively – demonstrates a strong ability to identify, analyze and solve problems.

Role models professional growth and development through participation in educational programs. Identifies needs and sets goals for own professional growth and development, meets all mandatory system/departmental educational requirements. Assimilates knowledge gained from conferences, workshops, seminars, and professional literature into practice. Develops mechanisms that facilitate transmission of new knowledge to other team members.

Seek to make improvements in work and assure that those improvements align with departmental and organizational strategy.

Performs other duties as assigned or as needed to meet the needs of the unit(s)/organization.
