Position Summary

The Registered Nurse promotes and restores patient health by providing, coordinating, and evaluating comprehensive nursing care through the application of the nursing process. This role supports patients of all ages, backgrounds, and cultures in a primary care setting.

Working as an integral member of the care team, the Registered Nurse collaborates with providers, medical assistants, and administrative staff to ensure timely, efficient, and coordinated patient care. This position requires strong clinical judgment, adaptability, and the ability to perform effectively in a fast-paced clinical environment.

Essential Duties & Responsibilities

  • Provide comprehensive nursing care using all phases of the nursing process, including assessment, planning, implementation, and evaluation
  • Assess patients through both phone triage and in-person visits using established clinical protocols
  • Collaborate with primary care providers to deliver appropriate patient care and coordinate escalation when needed
  • Work as part of the primary care team to manage patients from diverse age, cultural, and socioeconomic backgrounds
  • Involve patients, families, and appropriate healthcare providers in care planning when appropriate
  • Provide patient and family education based on individual needs, health literacy, and cultural considerations
  • Communicate clearly, concisely, and professionally with patients and care team members in both written and verbal formats
  • Maintain timely, accurate, and complete documentation in the electronic health record (EHR)
  • Work closely with medical assistants and administrative staff to ensure efficient coordination of care, scheduling, prescriptions, and paperwork
  • Demonstrate clinical and technical competence, including phlebotomy, point-of-care testing, medication administration, vaccinations, and injections
  • Administer medications safely using the “five rights” of medication administration
  • Maintain a safe and clean clinical environment through adherence to infection control and risk management practices
  • Oversee and support clinical workflows and delegate tasks appropriately to Medical Assistants and other support staff
  • Perform effectively and professionally in a fast-paced environment and during high-demand situations
  • Ensure compliance with all SBCHC policies, procedures, and regulatory standards, including completion of mandatory trainings
  • Participate in ongoing professional development and training opportunities
  • Perform additional duties as assigned
Requirements:
  • Degree from an accredited school of registered nursing (Associate’s or Bachelor’s degree)
  • Current Massachusetts Registered Nurse (RN) license in good standing
  • Strong clinical assessment and patient care skills
  • Ability to work effectively in a fast-paced, team-based healthcare environment

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN) preferred
  • Experience in a primary care, outpatient, or community health setting
  • Experience working with diverse or underserved populations
  • Bilingual skills preferred but not required

Physical Requirements

  • Ability to bend, lift, pull, and carry up to 70 pounds
  • Ability to sit, stand, and walk for extended periods
