ORGANIZATION SUMMARY
World Relief is a global Christian humanitarian organization whose mission is to boldly engage the worlds greatest crises in partnership with the church. Today, organizational programming focuses on humanitarian and disaster response, community strengthening and resilience, and refugee & immigrant services and advocacy.
POSITION SUMMARY:
This position will facilitate group classes for caregivers, assist program participants in gaining access to community resources, and work with volunteers to help clients adjust to life in the U.S. Additionally, the position will focus on supporting case managers as well as provide Medicaid funded case management service. A successful candidate will demonstrate a proven ability to assist immigrants in achieving stability and overcoming barriers that hinder healthy integration. This position is grant funded.
ROLE & RESPONSIBILITIES:Group Services
- Facilitate topic-specific group sessions for a specific number of individuals in caregiving roles annually, covering topics such as self-care, health promotion topics, psychosocial education, and caregiving resources. The focus on groups will be to provide support and community connection for those who are caregivers for older adults, disabled children, or disabled adults living in the household.
- Develop curriculum and pre/post tests for each topic presented
- Work closely with volunteers providing support in the form of transportation, childcare, materials, or specific facilitation for group sessions.
- Collaborate with the Services to Older Refugee (SOR) team to provide meaningful group services that include both caregiver and older adult.
- Meet with ethnic community leaders to assess needs and promote group programming
- Collaborate and provide training for community partners, mutual aid associations, and churches
- Collaborate with appropriate WR staff around service provision as necessary
Client Services
- Conduct plan of care and assess needs, strengths, barriers, and set goals with each participant receiving individual case management and checking in at least once every 30 days to ensure goal achievement.
- Work with individuals funded through Medicaid to provide case management supportive services. For Medicaid funded activities, conduct all necessary assessments and follow up through IM+CANS.
- With the goal of 3-9 months, exit client once all goals have been completed, conduct assessments and close out plan of care
- Regularly assist other case specialists with case management tasks when capacity allows
- Complete other duties as assigned
Administrative
- Write detailed case notes for each task/client interaction. Keep track in two different systems based on client service. Ritten is used for Medicaid and Dynamics for everything else
- Retrieve the signatures of each participant and complete all necessary program paperwork
- Write monthly stories and provide input for grant reports
- Research topics and gather information for group sessions and the effectiveness while utilizing evidence-based practices and peer reviewed materials Organizational Responsibilities
- Attend all department, team, and staff meetings as scheduled
- Attend regular Medicaid meetings organized by WR Counseling Center held off-site to discuss cases and other topics related to mental health
- Work alongside other staff to conduct seasonal or special events
- Mature and personal Christian faith
- Committed to the mission, vision, and values of World Relief
- Desire to serve and empower the Church to impact vulnerable communities
- Able to affirm and/or acknowledge World Reliefs Core Beliefs, Statement of Faith, Christian Identity and National Association of Evangelicals' For the Health of The Nation document
- Valid drivers license, access to reliable vehicle and ability to pass MVR standards
- Ability and willingness to drive 15-passenger van
- IM+CANS Certified or willingness to become certified
- Regular travel is required (client’s home and aiding in the connection to other service providers and group sessions)
- Occasional evenings or weekends required
- Work and supervise interns
- Strong Microsoft Office skills required
- Four-year degree from an accredited University preferred
- Self-motivated and able to work independently and handle a variety of tasks simultaneously
- Able to serve sensitively and effectively in a cross-cultural setting; cross-cultural experience desired
- Detail-oriented and strong organizational skills
- Ability to collaborate and coordinate with a team of service providers to help clients achieve goals
- Strong follow-through that focuses on both the start and finish of projects or client goals
- Ability to appropriately handle conflict and willingness to engage in mediation with clients
- Fluency in Spanish, Arabic, Dari, or Swahili is strongly preferred
- Strong oral and written English communication ability
