Healthcare Coordinator – Campus Adult and Children Overview
Responsible for the direction and overall delivery of healthcare and medical services for the assigned children and adult Campus Programs. Gives direct supervision to all Primary Care Nurses (PCN) and shift nurses for assigned Campus Programs. Coordinates all the systems for the medical service and health care needs of the individuals receiving services at the assigned Melmark Campus homes. Duties and responsibilities include the provision of excellent healthcare services for all residents receiving services in the assigned. Campus Programs 24 hours a day.
Schedule and Salary Information
- Fully on- site
- Schedule: Monday- Friday 8am-4:30pm
- On call rotation
- Starting Salary: $89,440.00 (+) based on years of experience
Requirements
- Current PA nursing license required. Registered Nurse (RN) required.
- Must hold a US Driver’s License
- Knowledge of regulatory standards for PA’s ODP 6600, 6100, and 6400regulations. Knowledge of Children's 3800 regulations preferred.
- Strong interpersonal skills, good writing, communication and organizational skills.
- Two years of direct practice experience required.
- Experience in supervising employees highly preferred.
Healthcare Coordinator – Campus Adult and Children Responsibilities
- Supervises the Primary Care (PCN) and shift nurses in the adult areas
- Assists Community or Melmark physicians in the establishment and implementation of quality health care for Melmark individuals.
- Ensures proper diets for all individuals by working closely with Melmark’s nutritionist and Speech Language Pathologist. Ensures diets are posted and annual nutritional evaluations are completed.
- In conjunction with the PCN, coordinates with Primary Care Physician (PCP) for proper medical care and wellness of all individuals. Identifies and ensures treatment of communicable diseases and reports all infectious conditions to appropriate personnel.
- Participates in HCC on-call rotation.
- Ensures all required medical paperwork is completed (Quarterly , Assessments, Compliance Trackers, HRT reports, etc). Provides regular reviews of medical records (appointments, lab results, current proper diagnosis)
- Provides comprehensive admission and discharge health care information.
- Attends weekly management meetings. Attends weekly IDT meetings. Attends HRT and ISP meetings as needed and delegated by DOH/SDOH.
- Responsible for oversight of completion of medical appointments (scheduling appointments and posts weekly appointment schedules) and completion of medical
- Responds to all codes and emergencies
