The Licensed Practical Nurse (LPN) Care Coordinator provides comprehensive care coordination services to FellowshipLIFE members and clients across home and community-based settings. and serves as the primary point of contact for members and families, ensuring seamless coordination of healthcare services, community resources, and interdisciplinary communication to promote safe, high-quality, person-centered care.
ESSENTIAL FUNCTIONS:
- Coordinate services across the healthcare continuum to ensure timely, efficient, and member-centered care.
- Serve as the primary contact for assigned members and caregivers.
- Assist the RN Care Manager with implementation and monitoring of individualized care plans.
- Perform ongoing telephonic and in-person follow-up with members to monitor changes in condition and service needs.
- Coordinate physician appointments, specialist referrals, diagnostic testing, transportation, errands, and community resources.
- Facilitate smooth transitions following hospitalization, emergency department visits, skilled nursing stays, rehabilitation, or hospice services.
- Assist members and families in navigating healthcare services and available FellowshipLIFE programs.
- Identify barriers to care and communicate concerns promptly to the RN Care Manager.
ADDITIONAL RESPONSIBILITIES:
- Attend required meetings, case conferences, and continuing education.
- Travel to FellowshipLIFE communities and member homes as needed.
- Participate in rotating on-call coverage if needed.
- Maintain professional licensure and continuing education requirements.
- Demonstrate flexibility during periods of high census or organizational need.
- Perform other duties as assigned.
MINIMUM ACCEPTABLE QUALIFICATIONS:
- Current unrestricted Licensed Practical Nurse (LPN) license in New Jersey.
- Minimum two (2) years of clinical nursing experience.
- Experience in one or more of the following: Home Health, Care Management, Case Management, Geriatrics, Community-Based Services, Primary Care, Assisted Living
- Valid New Jersey driver's license with reliable transportation.
Preferred Experience:
- Experience coordinating care for older adults.
- Experience with Medicare, Medicare Advantage, Managed Care, or chronic disease management.
- Knowledge of community resources available to older adults.
- Experience with electronic medical records and care management platforms.
Knowledge, Skills & Abilities:
- Strong communication and relationship-building skills.
- Excellent organizational and time-management abilities.
- Ability to prioritize multiple tasks in a fast-paced environment.
- Strong customer service skills.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
- Critical thinking and problem-solving skills.
- Proficiency in Microsoft Office and electronic health records.
- Demonstrated commitment to person-centered care.
