Home Care Liaison in Reynoldsburg, Ohio at Scarlet Home Health Care

Scarlet Home Health Care is seeking an energetic and relationship-driven Home Care Liaison to join our team! This role is essential in building strong partnerships with hospitals, inpatient rehab facilities, skilled nursing facilities (SNFs), and assisted living communities to support seamless transitions of care for patients returning to their homes.

The Home Care Liaison is responsible for growing referral sources, educating the medical community about our services, and ensuring smooth coordination between facilities and our clinical team. This is a field-based role that covers the Greater Columbus area.

This is an on-the-road position that will cover the east side of the Greater Columbus area.

What You'll Do

• Achieve and exceed monthly admission goals through outreach and account development.
• Develop and maintain strong relationships with referral sources, including hospitals, SNFs, rehab centers, and senior living communities.
• Conduct regular in-person sales calls and follow-up visits to promote Scarlet Home Health Care’s services and specialties.
• Coordinate timely and appropriate referrals with internal staff including the intake and clinical teams.
• Educate medical professionals and community partners on the features and benefits of home health services, including Medicare guidelines and specialty programs.
• Act as a point of contact for referral sources, ensuring prompt follow-up and excellent customer service.
• Participate in marketing events, community outreach, and in-service education sessions as needed.
• Submit timely administrative reports including call logs, weekly schedules, time sheets, and expense reports.

What You'll Need

• Bachelor’s Degree in Business, Marketing, Healthcare, or a related field preferred.
• 1–3 years of healthcare marketing or sales experience; home health or post-acute care experience preferred.
• Excellent interpersonal, communication, and relationship-building skills.
• Professional appearance and demeanor; able to represent Scarlet Home Health Care in the community.
• Must have a valid driver’s license, reliable transportation, and maintain auto liability insurance.
• Highly organized, self-motivated, and comfortable working independently in the field.
