Medicine Reconciliation Tech I at Shannon Health – San Angelo, Texas

Job Summary

Performs functions of pharmacy technician under supervision of pharmacist. Review, clarify, document, and verify with the patient, if possible, their medications taken prior to admission to the hospital. The Medication Reconciliation Technician will utilize the patient’s pharmacy, family, caregiver or prior office records if needed to further investigate and verify medications on, to be added, or to be removed from the home medication list in the hospital's electronic medical record. The technician will keep records of all work done for medication reconciliation performance monitoring.

Qualifications

Education

• Required
  • High School Diploma, GED, or equivalent

Experience:

• Preferred
  • Six months or more pharmacy technician experience

Certification/Licensure:

• Required
  • Registered Pharmacy Technician from the Texas State Board of Pharmacy (TSBP)
  • Certified Pharmacy Technician from Pharmacy Technician Certification Board (PTCB)
  • Valid Texas Driver's License
