What You'll Do (Your Role & Impact)
- Deliver Compassionate Care: Carry out personalized plans of care directed by physicians to keep patients safe and comfortable at home.
- Prevent Re-Hospitalizations: Actively monitor condition changes to adjust care plans early and minimize hospital visits.
- Collaborate on Care: Communicate directly with physicians regarding medication needs and ongoing care adjustments.
- Empower Families: Teach and counsel patients and family members on managing injuries, diseases, and recovery processes.
- Digital Documentation: Complete patient care records efficiently in our WellSky EMR system.
- Continuous Growth: Attend orientation, ongoing staff meetings, and earn CEUs.

Requirements:What We’re Looking For
- Active OH LPN License (in good standing)
- Current CPR Certification
- Reliable Transportation & current auto insurance
- Tech Comfort: Solid computer skills and ability to navigate an EMR system (WellSky experience is a plus!)
- Industry Knowledge: Working understanding of Medicare/Medicaid guidelines for Home Health
- Key Traits: Strong independent work ethic, a compassionate heart, and a passion for working with homebound patients
