POSITION SUMMARY

The Senior Services Care Coordinator functions on behalf of the skilled nursing facility and the residents to attain or maintain the highest practicable physical, mental and psychosocial well-being of each resident.

POSITION QUALIFICATIONS

Minimum Education

Licensed Practical Nurse or Bachelor of Social Work

Minimum Experience-
2 years in nursing or medical social work

Required Registration/License/Certification-
Active Alabama license by State Board of Social Work or Nursing

Preferred Education-
BSN or MSW

Preferred Experience-
5 years nursing or social work experience.
