Responsibilities and Duties: $3,000 Sign On Bonus • Develops and revises plan of care in consultation with physician and other care-team members. • Provides therapeutic treatments and evaluation of equipment needs as applicable. • Participates in clinical conferences and in-service programs. • Conducts on-site supervisory visits of physical therapy assistants. • Advises and consults with the family and other agency personnel as applicable. • Prepares clinical and progress notes. • Protects and honors patient and coworker confidentiality and right to privacy. • Evaluates outcomes of care. • Plans for discharge.
