Job Summary

Performs assessments of patient activity needs, develops a plan of care based on the assessment and implements the plan in conjunction with the patient and/or family. Participates as part of the interdisciplinary team.

Qualifications

Education

• Preferred
  • High School Diploma, GED, or equivalent

Experience:

• Required
  • Completed two years in a social or recreational program within the last five years, one of which was full-time in a patient activities program in a health care setting or has completed an activity training course approved by any state.

Certification/Licensure:

• Required
  • Activities Director Certification
    • Must obtain within one (1) year of start date
  • Basic Life Support (BLS) Certification
    • Must obtain within thirty (30) days of start date
  • Safe Approach Level III Certification
    • Must obtain within six (6) months of start date
