About the Role
The Service Coordinator is responsible for coordinating and delivering Triple C's General Services Model at assigned affordable housing developments through contractual partnerships and Memoranda of Understanding (MOUs). The position serves as an onsite resource for residents, connecting individuals and families with community-based services, educational opportunities, wellness activities, and other supports that promote housing stability, self-sufficiency, and community engagement. The Service Coordinator is accountable for maintaining the required onsite presence at each assigned property, planning and executing resident programming, providing appropriate outreach and resource coordination, and maintaining accurate documentation and reporting.
Successful performance requires consistent fulfillment of each property's contractual service requirements, including a minimum of three resident events or structured programs per month at each assigned property, unless a higher requirement applies. The position may support multiple affordable housing developments and requires independent time management, professional communication, effective community partnerships, and consistent accountability to the Partnership Programs Supervisor.
Key Responsibilities

- Develop and maintain a regular property-specific schedule that supports resident accessibility, engagement, programming, and service delivery.
- Arrive as scheduled, remain onsite for assigned service hours, and promptly communicate anticipated absences, coverage gaps, or scheduling changes to the Partnership Programs Supervisor.
- Understand and fulfill the specific service commitments, activities, and deliverables associated with each assigned property.
- Accurately record all hours worked, including onsite services, events, required meetings, administrative activities, and compensable travel, in accordance with agency timekeeping policies.
- Travel between assigned developments and community locations as necessary to fulfill program responsibilities.
Resident Programming & Community Engagement
- Plan, coordinate, promote, execute, and document a minimum of three resident events or structured programs per calendar month at each assigned property, or more when required by the applicable MOU or contract.
- Develop meaningful programming responsive to resident interests, identified needs, and Triple C's General Services Model, including financial literacy, health and wellness, employment readiness, independent living skills, tenant education, social engagement, and community integration.
- Coordinate community organizations, educational presenters, volunteers, service providers, and other external partners to deliver relevant programming and activities.
- Develop and distribute event calendars, flyers, announcements, and other outreach materials to promote resident participation.
- Maintain documentation of completed events, including dates, topics, promotional materials, attendance, participating partners, and resident feedback.
- Monitor resident participation and engagement trends and adjust outreach and programming strategies to improve accessibility, relevance, and participation.
Resident Support & Resource Coordination
- Engage residents to identify supportive-service needs and provide information, assistance, referrals, and connections to appropriate community resources.
- Support resident access to financial assistance, public benefits, healthcare, employment services, educational opportunities, transportation, food resources, and other community-based supports.
- Complete and document at least one monthly check-in or outreach attempt with each assigned set-aside household, where required by the applicable program expectations, and provide appropriate follow-up.
- Conduct general needs assessments, assist with individualized goals, and coordinate referrals or follow-up services as required by the applicable MOU or approved service model.
- Promote housing stability by helping residents access resources related to independent living, landlord-tenant relationships, household management, and other identified needs.
- Collaborate with Property Management regarding resident concerns affecting housing stability while maintaining appropriate confidentiality, professional boundaries, and the respective responsibilities of each organization.
- Identify and promptly elevate significant resident concerns, safety risks, unmet needs, or service barriers through established supervisory and organizational procedures.
Community Partnerships & Professional Communication
- Establish and maintain collaborative relationships with Property Management, developers, community organizations, healthcare providers, municipalities, and other external partners.
- Represent Triple C Housing professionally at assigned developments, resident activities, community meetings, and other approved events.
- Maintain regular communication with the Partnership Programs Supervisor regarding site activities, schedules, resident engagement, upcoming programming, external relationships, and outstanding responsibilities.
- Participate in scheduled supervision, team meetings, professional development, and other required agency activities.
- Communicate contractual concerns, partner complaints, service-delivery challenges, and other significant issues promptly through the established chain of command.
- Maintain appropriate professional boundaries and obtain supervisory approval before making commitments or representations outside the position's delegated authority.
Documentation, Reporting & Performance Accountability
- Independently prepare and submit accurate, complete, and timely monthly service reports for each assigned Partnership Program property.
- Document required and completed onsite hours, resident outreach, set-aside household contacts, completed events, attendance, referrals, community partnerships, identified needs, outcomes, and other applicable contractual deliverables.
- Maintain organized supporting records, including schedules, calendars, flyers, attendance records, outreach documentation, and referral information sufficient to verify services delivered.
- Monitor completion of assigned contractual requirements and promptly communicate any barriers that may affect onsite coverage, programming, resident engagement, or reporting.
- Participate in program monitoring, internal reviews, quality improvement, audits, and other activities intended to strengthen contractual performance and resident outcomes.
- Maintain accurate, confidential records in accordance with agency policies, applicable laws, and program requirements.
