Job Description

Job Summary:

The position requires that the individual assist the professional nursing staff by performing specific nursing functions related to patient care under the supervision of a registered nurse or therapist and for the secretarial aspects of patient care and the environmental management of the work unit. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served; also demonstrates knowledge of the principles of growth and development and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his/her age specific needs, provide reliable assistance to the members of the team including nurses, manager, and physicians. Provides the care needed as described in the department's policies and procedures. Demonstrates good customer service skills when interacting with patients, visitors, and co-workers. Helps ensure that the unit runs efficiently. Keeps charts organized. Assists with team meetings. Performs Certified Nursing Assistants duties as staffing needs require and other delegated duties to promote unit running efficiently.

Performance Expectations:

  • Demonstrates the ability to order supplies, nourishments prior to needing them.
  • Demonstrates the ability to navigate the electronic medical record to document appointments made to physicians for our residents.
  • Demonstrates the aptitude to navigate the electronic medical record for other roles, such as aide responsibilities.
  • Demonstrates excellent telephone skills and shows that she is a good example to others to follow.
  • Assists the team with ensuring the residents get to the team meetings once a week.
  • Demonstrates the ability to answer the call lights and keep a log of the calls.
  • Demonstrates the ability to keep the unit clean and organized, and ensuring the refrigerator is kept clean and temps are within range.
  • Demonstrates the knowledge and skills necessary to collect data and provide care appropriate to the age of the patients served; also demonstrates knowledge of the principles of growth and development and possesses the ability to read data reflective of the patient’s status and interprets the appropriate information needed to identify each patient’s requirements relative to his/her age specific needs, and to provide the care needed.
  • Demonstrates the aptitude to navigate the electronic medical record to document the daily care of the residents, to include vitals, I and O’s, ADL’s, Turns, Rounds, Weights, etc.
  • Demonstrates good bedside and customer service skills.
  • Demonstrates the knowledge of the handbook regarding the employee responsibility regarding attendance, benefits, attitude, cell phones, and etc.
  • Adheres to and promotes the established values of the organization, i.e. customer service, safety, infection control and compliance standards and ensures that unit runs efficiently.
  • Demonstrates thorough, concise and timely documentation and bedside shift reporting. Communicates pertinent patient information to all team members; under direct supervision their Registered Nurse or Licensed Practical Nurse.
  • Demonstrates effective communication with others in an appropriate and timely manner. Embraces and practices We CARE philosophy in all aspects of patient, visitor, co-worker, employee and non-employee care.
  • Maintains own clinical skills staying current in skills. Maintains annual competencies for post-acute skilled nursing.
  • Demonstrates the ability to perform all skills required for the Skilled Nursing unit’s population by rendering professional certified nursing assistant care including, but not limited to measuring vital signs; including manual, perineal care, Foley catheter care, incontinent care, oral and dental care, feeding patients, managing patient hygiene, bathing and repositioning of patients, conducting timely rounds, obtaining weights, etc.
  • Demonstrates the aptitude for working and troubleshooting equipment used including but not limited to Dina map (vital signs machines), manual blood pressure, specialty beds, repositioning of hospital bed, bedside table, wheelchairs, Hoyer lifts, standing lifts and recliner and etc.…
  • Demonstrates the aptitude for safely positioning and assisting patients using a variety of devices such as but not limited to pillows, walkers, crutches and etc.
  • Demonstrates critical thinking skills and uses professional judgement to respond promptly with intervention when clinical changes occur and report any changes promptly to their nurse (direct report).
  • Demonstrates the knowledge of abuse/neglect of vulnerable persons.
  • Demonstrates the knowledge of educational opportunities/in-service education and webinars regarding the type of unit.
  • Consistently follows FGH Skilled Nursing Policies, Patient Care Services and FGH policy, procedures, and standards of care in performing nursing duties.

Qualifications:

Education/Skills

High School Diploma/GED and completion of an approved Nurse Aide training course required. Successful completion of technician course at FGH during clinical orientation required.

Work Experience:

Nurse Aide work experience required. Computer experience preferred.

Certification/Licensure-DUE UPON HIRE

  • Certified Nurse Aid Certificate

Additional Certification/Licensure - Obtained based on required timeframe below

  • Basic Life Support
