Position Summary

The Clinical Manager is responsible for overseeing day-to-day clinical operations, supporting field staff, and ensuring patient care is delivered safely, effectively, and in compliance with physician orders, agency policies, and regulatory requirements.

This role provides direct support to clinical staff, assists with case management oversight, and works closely with leadership to maintain quality, efficiency, and continuity of care.

Reporting Structure

This position reports to the Director of Nursing (DON) and works closely with the Administrator and interdisciplinary leadership team. The Clinical Manager provides supervision and support to field clinicians including Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), and therapy staff as assigned.

Scope of Practice

This role is clinical and administrative in nature and functions within the scope of Florida RN licensure. The Clinical Manager supports patient care coordination, staff supervision, and clinical oversight but does not replace the responsibilities of the Director of Nursing.

Essential Job Functions

1. Clinical Oversight & Support

  • Supervise and support field clinicians in delivering patient care according to care plans and physician orders
  • Provide guidance on clinical questions, patient care concerns, and care plan implementation
  • Monitor patient care delivery to ensure quality, safety, and compliance standards are met
  • Support continuity of care across interdisciplinary teams

2. Staffing & Field Coordination

  • Assist with clinical staffing needs in coordination with the staffing department
  • Support assignment of clinicians based on patient needs, skill level, and geographic coverage
  • Address field staff concerns, schedule issues, and visit coordination challenges
  • Ensure timely response to clinical escalations or patient care issues

3. Quality & Compliance Support

  • Support compliance with Medicare Conditions of Participation, state regulations, and agency policies
  • Assist with preparation for audits, surveys, and regulatory inspections
  • Review clinical documentation for completeness and accuracy as needed
  • Support implementation of corrective action plans and quality improvement initiatives

4. Communication & Care Coordination

  • Serve as a clinical point of contact for field staff, patients, and interdisciplinary teams
  • Communicate patient updates, concerns, and care plan changes to appropriate team members
  • Participate in care coordination with physicians, therapists, intake, billing, and leadership teams

5. Staff Development & Leadership

  • Support onboarding and orientation of new clinical staff
  • Provide ongoing coaching, mentoring, and performance feedback to field clinicians
  • Participate in training, in-services, and competency evaluations
  • Promote a culture of accountability, teamwork, and high-quality patient care

Qualifications

  • Bachelor’s degree in Nursing (BSN required; MSN preferred) or related healthcare field required. Current Registered Nurse (RN) license in good standing required. Minimum of three (3) years of clinical nursing experience required, with at least two (2) years in home health and one (1) year in a supervisory or leadership role required.
  • Strong knowledge of home health regulations and Medicare Conditions of Participation
  • Experience with EMR systems such as Axxess, WellSky, (Kinnser), or similar preferred
  • Current CPR certification required
  • Valid driver’s license, reliable transportation, and auto insurance
  • Ability to pass background screening and all compliance requirements

Physical Requirements

  • Ability to sit, stand, and work in both office and field environments as needed
  • Frequent use of computer systems, EMR platforms, and communication tools
  • Ability to travel to patient homes and field locations as required
  • Ability to review and analyze clinical documentation and reports
  • Effective communication with staff, patients, families, and leadership

Exposure to variable environmental conditions may include pets, smoke, infectious diseases, and weather-related travel conditions.
