POSITION SUMMARY

The Patient Navigator is a healthcare professional that provides supportive counseling and referral information for patients and families, and performs complete psychosocial assessments as well as mental health and substance abuse screenings. The Navigator is committed to removing the patients barriers to care by identifying critical resources for clients, helping them through health care services and systems and promoting patient health. The Navigators demonstrates understanding of community resources, communicates, and collaborates effectively with agencies and hospital personnel. The Navigator is also responsible for writing medical record notes within 48hrs, and building constructive relationships with community service professionals. This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to teamwork and cooperation.

QUALIFICATIONS EDUCATION: Bachelors degree in Social Science required.

LICENSE/CERTIFICATION: EXPERIENCE: Minimum of 2 years experience in case management required. Excellent written and verbal communications skills required. Bilingual preferred. Demonstrates ability to work with diverse populations. Proficiency in Clinical Applications preferred at time of hire; incumbents within position will be trained appropriately and then skill will be required for this position within 30-60 days from date of hire.

PERFORMANCE EXPECTATIONS Demonstrates the competencies as established on the Assessment and Evaluation Tool for this position.

ENVIRONMENT Potential for exposure to the hazards and risk of the ambulatory and hospital environment, including exposure to infectious disease, hazardous substances, and potential injury. The essential functions for this position are listed on the Assessment and Evaluation Tool.

REPORTING RELATIONSHIP This position reports to department leadership.
