Social Services Designee at Citizens Memorial – Bolivar, Missouri

Citizens Memorial Healthcare Foundation (CMH) is recruiting for a Social Worker/Social Service Designee to join the dynamic team of providers already in place providing exceptional patient care at the Parkview Healthcare Facility.

The skilled nursing facility is an 78-bed facility located in Bolivar, MO that includes a memory care unit for individuals with Alzheimer's disease and dementia.

A typical day will include: social service management for the 18-bed skilled nursing unit. You will stay with residents throughout their stay at the facility and will be responsible for assisting with completion of the admission process and working with the interdisciplinary team on discharge planning.

Position is: Full Time, Days 8:00a.m-4:30p.m

Education: Bachelor’s degree in Social Work or related field required
