Navigator in San Antonio, Texas at University Health System- San Antonio

This position will serve as the link between the community, health system providers, external providers, social workers, network partners and the Ryan White Care Coordination team in order to increase the utilization of available health promotion programs and maximize the use of existing health services as deemed appropriate. Assists and guides patients through community health prevention and treatment services to ensure that patients are able to access services and keep medical appointments, participate in recommended care plans, and receive appropriate health information about their screening results, including next steps. Assists and supports the department’s efforts in community venues including, but not limited to, health fairs, sponsored events, symposiums, and educational classes provided to the community within the service region. All functions and behaviors are applicable to clinical and community settings.

EDUCATION/EXPERIENCE

A Bachelor’s degree in social sciences or health education is the preferred or related field of study.

A minimum of 2 years or equivalent experience in the area of health education, public health,

or case management. Experience in public speaking and educational presentations

demonstrating effective communication skills is required. English/Spanish fluency is strong

desired.
