Nurse Case Manager (RN) – Hospital to Homes Scarborough Community Treatment Order Program (Full Time – 5 Months)

The Role and Opportunity


The Canadian Mental Health Association (CMHA) – Toronto is seeking a dedicated and compassionate Nurse Case Manager (RN) to join our Hospital to Homes (H2H) Program. This intensive case management program supports individuals with severe mental illness who are under a Community Treatment Order (CTO). The program operates in partnership with Scarborough Health Network (SHN) and Lakeridge Health, with referrals primarily from SHN.

Grounded in psychosocial rehabilitation principles, our program emphasizes recovery through client collaboration and engagement. Services are community-based and provided across diverse settings in Scarborough and East Toronto.

Salary:$63,879– $69,994+ 4% vacation Hours of Work:35 per weekLocation: East Office (Markham & Ellesmere) and throughout Scarborough/East TorontoAbout CMHA Toronto

Responsibilities

Build and maintain therapeutic relationships with clients and their support networks.Attend inpatient rounds at SHN’s Centenary, Birchmount, and General sites.Complete assessments, develop interprofessional care plans, and monitor care effectiveness.Provide psychoeducation on mental illness, medications, and symptom management.Conduct physical and mental status assessments and behavioral observations.Collaborate with internal teams, SHN, and community agencies to coordinate care.Link clients with appropriate community and health resources.Apply best practices in crisis intervention, trauma-informed care, and recovery-oriented support.Participate in the Nursing Quality Assurance (NQA) Committee.Contribute to primary care and chronic disease initiatives across the agency.
Experience & Education Required

BScN and current unrestricted registration with the College of Nurses of Ontario (CNO).CNA certification in Psychiatric/Mental Health Nursing (or in progress) preferred.Minimum 2 years’ experience working with individuals with mental illness, preferably in a community setting.Strong skills in mental health assessment, medication monitoring, case management, and crisis intervention.Proficient in Microsoft Office applications (Teams, SharePoint).Demonstrated leadership, problem-solving, and organizational abilities.Understanding of recovery-oriented, anti-oppressive, and culturally competent practices.Experience with addictions and dual diagnosis considered an asset.Fluency in a second language is an asset.Valid Ontario Driver’s License, access to a vehicle, and insurance are required.Availability to work primarily during the day, with occasional evenings.
