As a Registered Nurse, you’ll provide quality and compassionate care to individuals in their homes. Our team of Nurses are dedicated to making a difference where it matters most: at home. What You’ll DoClinical Responsibilities:
- Provide quality nursing care, within the scope of practice and in accordance with the CNO
- Maintain knowledge and compliance to the Professional Standards
- Follow nursing processes and practices in an effective and efficient manner
- Ensure the maintenance of high-quality nursing skills, applying current knowledge to nursing care
- Comply with all SJHC nursing practices, processes and work instructions
- Review and follow the plan of nursing care, including discharge plans
- Engage in consultation / liaison with clients, their families/caregivers, physical, multi-discipline team member, and other external stakeholders as required (eg. HCCSS, ICC Care Coordinators)
- Evaluate and monitor care plans on an on-going basis
- Assist other clinical programs if requested or required
- Participate in regular and on-going professional development / learning opportunities, with the goal of obtaining more advanced or specialized nursing skills, as may be necessary for organizational advancement
- Complete self-appraisals in keeping with the College of Nurses of Ontario Standards of Practice and SJHC internal practices
- Act as an advocate for patients, families and their caregivers through reviewing and interpreting information, supporting decision making and facilitating communications
- Complete comprehensive nursing assessments
- Conduct virtual care visits as necessary to program and/or scope of practice
Operational:
- Complete program, clinical or organizational documentation in a timely and accurate manner
- Participate in organizational learning opportunities as appropriate (e.g, Surge Learning)
- Engage in organizational working groups as appropriate (e.g, JH&SC, Quality Committee)
- Effectively utilize organizational tools, software, devices in the application of daily work, willingness and ability to learn new technical skills as operational or patient needs change
- Willingness and ability to provide care in a variety of geographical locations (eg. Central Hamilton), patient care environments (eg. apartment building), or institutional care settings (eg. Long Term Care)
- Willingness and ability to act as coach/mentor/Preceptor to new nurses through knowledge sharing and peer-to-peer support
Education & Credentials:
- Bachelor’s of Science in Nursing
- Registered with the College of Nurses of Ontario (CNO)
- Current BCLS/HCP certification
Experience:
- 1–3 years of clinical experience (required)
- Experience with IV medication administration (required)
- Home and community care experience (preferred)
- Familiarity with mobile work tools and EMR systems
