Position Overview:
We are seeking a warm, organized, and detail-oriented First Impressions Coordinator to join our client focused care team. This person will play a key role in ensuring our clients and their families have a seamless, personalized, and consistently positive experience with Coastal Care Partners.
Key Responsibilities:
- Serve as a primary point of contact for our medical and home care clients.
- Proactively check in with clients to ensure satisfaction and quickly address any concerns
- Manage care transitions and updates, including changes in client condition or care plan
- Maintain accurate client records and communication notes in multiple systems
- Support the onboarding of new clients and help create personalized care experiences
- Collaborate closely with medical providers, leadership, caregivers and internal teams to ensure quality and consistency

- Strong organizational skills and attention to detail
- Experience in customer service, home care, healthcare, or a related field
- Ability to juggle multiple tasks and stay calm under pressure
- A team player who thrives in a fast-paced, people-focused environment
- Comfortable with scheduling tools, databases, and phone-based communication
Bonus If You Have:
- Previous experience working in a home care, medical office, or senior care setting
- Knowledge of scheduling systems (e.g., AxisCare/ClearCare/WellSky/Athena or similar platforms)
- A background in nursing, care coordination, or case management
Location:
Bluffton, SC (in-office)
Schedule:
Full-time | Monday–Friday | Occasional on-call or after-hours support as needed
