About This Position

GENERAL SUMMARY:

Responsible for assessing the patient’s health status prior to admission, analyzing that information in conjunction with clinical data (lab and clinical testing results and physicians’ orders) and intervening to prevent problems that could result in adverse patient outcomes or delayed/cancelled procedures. Collaborates with multidisciplinary staff to initiate care planning needed during and after hospitalization/procedure as related to the special needs of the patient.

PRINCIPAL JOB FUNCTIONS:

1. *Commits to the mission, vision, beliefs and consistently demonstrates our core values.

2. *Initiates assessments via pre-admission phone contact or upon admission based on the patient’s presenting problems/needs in accordance with Grand Island Regional Medical Center’s procedures; conducts initial assessment and documents health history and current medications.

3. *Obtains and reviews physician’s orders, clinical testing/lab results and Health & Physicals (H&Ps); synthesizes pertinent patient data to identify priority needs to be addressed prior to or upon admission.

4. *Identifies potential problems with abnormal lab results, medications, pre-op orders, etc. and intervenes as appropriate with primary care physicians, surgeons, or anesthesia personnel.

5. *Collaborates with members of ancillary services in a multidisciplinary approach to prepare for patient admission, i.e. special needs for patients with latex allergies; works with Health Information Management to obtain H & Ps;

works with other departmental staff to have the patients arrive in time for required tests to be done or tests read prior to the scheduled surgery/procedure start time.

6. *Communicates special care needs to appropriate staff, expeditors and Managers to ensure and enhance patient care and satisfaction.

7. Identifies post-discharge care needs and initiates planning/referrals as appropriate.

8. *Provides pre-operative education for the patient or family to meet identified patient needs according to their needs, abilities, preferences, and readiness to learn; provides basic post-op education as appropriate.

9. Incorporates cultural considerations into patient’s plan of care.

10. *Locates required health records and documents patient care and related information according to Grand Island Regional Medical Center’s procedures for documentation.

11. Performs admission assessments and post-operative monitoring as needed.

12. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.

13. Participates in meetings, committees and department projects as assigned.

14. Performs other related projects and duties as assigned.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:

1. Maintains clinical competency as required for the unit including but not limited to age-specific competencies relative to patient’s growth and developmental needs, annual skill competency verification and mandatory education and competencies.

2. Knowledge of federal and state regulations related to nursing applications and pre-admission and pre-operative care.

3. Knowledge of computer hardware equipment and software applications relevant to work functions.

4. Ability to communicate effectively both verbally and in writing.

5. Ability to exercise professional judgment, courtesy, and tact in daily interactions with others.

6. Ability to adapt work pace according to rapidly changing volumes, patient needs, and unpredictable situations.

7. Ability to establish and maintain effective working relationships with all levels of personnel and medical staff.

8. Ability to problem solve and engage independent critical thinking skills.

9. Ability to maintain confidentiality relevant to sensitive information.

10. Ability to prioritize work demands and work with minimal supervision.

11. Ability to maintain regular and punctual attendance.

EDUCATION AND EXPERIENCE:

Current Registered Nurse licensure from the State of Nebraska or approved compact state of residence as defined by the Nebraska Nurse Practice Act.

OTHER CREDENTIALS / CERTIFICATIONS:

Basic Life Support (CPR) certification required. Bryan Health recognizes American Heart Association (for healthcare professionals), American Red Cross (for healthcare professionals) and the Military Training Network.
