Endeavor Health Services is seeking a Licensed Mental Health Clinician to join our Safe Options Support (SOS) Team, delivering Critical Time Intervention (CTI) to individuals experiencing street homelessness.

This is not a traditional clinic role. You’ll work as part of a multidisciplinary outreach team, engaging individuals in the community and supporting them through the critical transition from homelessness to stable housing—ensuring they build the supports needed to remain housed long-term.

Your Impact

Using the CTI model, you will provide structured, time-limited support across key transition phases:

  • Engagement & Trust Building: Conduct assertive outreach in streets, shelters, hospitals, and community settings
  • Transition to Housing: Coordinate housing placement, complete SPOA and other applications, and support move-in planning
  • Stabilization: Provide short-term clinical support and strengthen connections to behavioral health, medical care, and community resources
  • Transfer of Care: Ensure long-term supports are in place so participants can maintain housing and independence

What You’ll Do

  • Deliver field-based outreach and engagement using trauma-informed, harm reduction, and person-centered approaches
  • Provide brief, targeted clinical interventions aligned with CTI phases
  • Collaborate daily with SOS teammates and community partners to coordinate care
  • Participate in hospital discharge planning and care transitions
  • Advocate for participants experiencing systemic barriers or healthcare inequities
  • Support housing stability through ongoing check-ins and problem-solving with tenants and providers
  • Supervise case managers and peer staff
  • Complete timely documentation and contribute to data-informed care

What You Bring

  • Master’s degree or higher in Social Work, Mental Health Counseling, Nursing, or Psychology
  • Active license (LMSW, LCSW, LMHC, RN, or equivalent)
  • Experience providing mental health and/or substance use services
  • Strong clinical assessment and engagement skills in community settings
  • Ability to collaborate effectively within a multidisciplinary team

Preferred:

  • Experience working with homeless or vulnerable populations
  • Familiarity with Western New York systems and resources

Compensation & Benefits

  • $55,276 – $68,776/year (based on experience and credentials)
